ABSTRACT

To systematically evaluate the clinical efficacy and safety of Danhong Injection combined with conventional therapy in improving diabetes mellitus complicated with coronary heart disease. Based on the online literature database(CNKI, Wanfang, VIP, PubMed, Web of Science, Cochran Library), the Chinese and English papers about the randomized controlled trial(RCT) of Danhong Injection in the treatment of diabetes mellitus complicated with coronary heart disease were searched comprehensively from the establishment of the databases to January 1, 2020. The papers were screened strictly according to the inclusion and exclusion criteria. Based on Jadad scale, the risk assessment of literature was carried out, and Meta-analysis was performed by STATA 12.0 software. Seventeen RCTs were included, involving 1 453 patients. The results of Meta-analysis showed that the combination of Danhong Injection and conventio-nal treatment could improve the clinical comprehensive effective rate(RR=1.47, 95%CI[1.38, 1.58], P<0.000 1), electrocardiogram(ECG) efficiency(RR=1.30, 95%CI[1.16, 1.46], P<0.000 1), efficiency of the angina pectoris(RR=1.41, 95%CI[1.25, 1.58], P<0.000 1), cholesterol level(SMD=-1.05, 95%CI[-1.95,-0.16], P=0.02), low-density lipoprotein(LDL) level(SMD=-0.50, 95%CI[-0.79,-0.21], P<0.000 1), coronary angina attack frequency(SMD=-3.71, 95%CI[-4.05,-3.36], P<0.000 1) and duration of angina pectoris(SMD=-2.96, 95%CI[-3.25,-2.66], P<0.000 1), with statistically significant differences. But the differences in fasting plasma glucose(FPG)(SMD=-0.19, 95%CI[-0.45, 0.08], P=0.16), plasma glucose of two hours after meal(2 hPG)(SMD=0.19, 95%CI[-0.11, 0.49], P=0.22), and high-density lipoprotein(HDL) level(SMD=0.10, 95%CI[-0.30, 0.49], P=0.62) after treatment were not statistically significant. Compared with the control group, there was no significant difference in adverse reactions(SMD=-2.96, 95%CI[-3.25,-2.66], P=0.75). The existing evidence shows that the combination of Western medicine and Danhong Injection can improve the clinical effect for diabetes mellitus complicated with coronary heart disease and has no obvious adverse reactions. However, due to the low level of overall literature evidence, high risk and some kind of publication bias, it still needs more high-quality randomized controlled trials and low-bias studies for further verification.

ABSTRACT

[Objective] Summing up the specific application of the three stages therapy of Professor XU Li in the treatment of cancer.[Methods]By copying the prescription and consulting the medical books of Professor XU, learning his clinical experience in treatment of cancer,summing up the core thinking of the three stages therapy and the experience of Chinese medicinal plant. [Result]By summing up the three stages therapy(assisting ,maintaining and palliative treatment),the special herb and attaching importance to develop the XIAN TIAN and HOU TIAN in the treatment of cancer ,it can improve radiation and chemotherapy therapeutic efficacy and resistance to transfer the maintenance treatment of recurrence ,improve the quality of life and prolong survival time of palliative care.By the guidance of the three stages therapy,smoothly finish the postoperative radiotherapy of nasopharyngeal carcinoma,successfully reverse the precancerous lesions of chronic atrophic gastritis with intestinal metaplasia,the two typical medical cases have significant curative effect. [Conclusion] Taking the three stages therapy in the process of the treatment of the tumor and precancerous lesions ,it can get good clinical effect,it is worth spreading in clinic.

ABSTRACT

<p><b>OBJECTIVE</b>This study aimed to evaluate the effort of applying frontal and occipital bones in extensive cranioplasty and preserving multiple cranial bone flaps adhered to the dura mater in the treatment of sagittal synostosis.</p><p><b>METHODS</b>From April 2008 to June 2013, sixty-three children with sagittal synostosis, aged 5 months to 3 years, were included in the study. The frontal bone flap was removed using an air drill. The occipital and bilateral temporal bone flaps were cut open but not detached from the dura mater or fixed to produce floating bone flaps. The skull bone was cut into palisade-like structures. Brain compression from both sides and the base of the skull was released and the brain expanded bilaterally through the enlarged space. Only a long strip-shaped bone bridge remained in the central parietal bone. Subsequently, the frontal bone flaps and occipital bone flap were pushed towards the midline and fixed with the parietal bone bridge to shorten the anteroposterior diameter of the cranial cavity and allow the brain to expand bilaterally to correct scaphocephaly. The CT images showed that both sides of the parietal bone of artificial sagittal groove gradually merged postoperative 1 year, and skull almost completely normal healing after operation 2 or 3 years, without deformity recurrence within 5 years. Among them all, 61 children's intelligence is normal and 2 children's lagged behind normal level, no further improvement.</p><p><b>RESULTS</b>Patients were followed up 1 - 5 years (an average of 43 months). Skull growth was excellent in all patients, the anteroposterior diameter was shortened by 14.6 mm averagely, the transverse diameter was increased by 12.3 mm averagely, the prominent forehead was corrected, and scaphocephaly improved significantly. There were no complications such as death and skull necrosis.</p><p><b>CONCLUSIONS</b>The application of frontal and occipital bones in extensive cranioplasty and preserving multiple cranial bone flaps adhered to the dura mater can be used in the treatment of sagittal synostosis. Surgery without removing bone flaps is less traumatic and results in no massive bleeding. It can effectively relieve brain compression and promotes transversal expansion of the brain during surgery and subsequent normal brain development.</p>

ABSTRACT

OBJECTIVE@#To explore the neuroprotective effect of pAkt and HIF-1 α on ischemia rats.@*METHODS@#The rat model of cerebral ischemia which induced by permanent middle cerebral artery occlusion was established, Silybin were given respectively. The behavior was measured by modified Longa method, brain water content were measured by the dry-wet method. Infarct volume was measured by image analysis method, Akt, HIF-1 α, Bcl-2, Bax, NF-κ B protein expressions were detected by Western blotting. The Akt, HIF-1 α, Bcl-2, Bax, NF-κ B mRNA expression were detected by RT-PCR.@*RESULTS@#The control group, low-dose silibinin group and high-dose silibinin group showed paralytic of the left body of rats in various degrees, the brain water content increased and different infarction size. There was no abnormal of the neurobehavioral assessment and no cerebral infarction in the blank group. Compared with the control group, there was no significant improvement of neurological function (t=1.341, P=0.188) or significant changes of the infarct volume (t=1.737, P=0.091) in the low-dose silibinin group, while there was significantly improvement of the neurological function in the high dose silibinin group (t=12.979, P0.05). In the high dose silybin group, the p-Akt 473, p-Akt 308, HIF-1α, Bcl-2 protein and Akt, Bcl-2 mRNA expression were increased, while the Bax, NF-κ B protein and Bax, NF-κ B mRNA expression were decreased, the differences were statistically significant (P0.05).@*CONCLUSIONS@#pAkt, HIF-1 α have neuroprotective effect on ischemia rats.
